Skilled Nursing and Rehabilitation Facility

Skilled Nursing Care

High quality care and a team approach are important to our skilled nursing program. Individual care plans with agreed-upon goals are designed by our interdisciplinary team, the resident and family.

Skilled nursing care is provided to the resident as ordered by the admitting physician and facility medical director.

Dementia and Alzheimers

Our highly trained staff cares for those requiring additional care due to dementia or Alzheimer's. We promote and follow established safety precautions for these residents.

Dietary

Our dietary department is managed by Morrison Health Food Services. All residents receive a nutritional screening assessment by our clinical dietitian following admission.

Meals are prepared fresh daily by our on-site dietary department.

Housekeeping and Laundry

We take pride in the cleanliness of our facility and all resident rooms are cleaned daily.

Laundry services are provided by Oakwood.

Activities

We offers unique approaches to social and recreational activities. Our certified recreational therapist designs daily activities that promote social and physical interaction, communication and fond memories.

Oakwood provides residents with transportation on outings, shopping trips and community activities.  

Social Services

The Social Services department at Oakwood offers each resident the option to discuss and develop solutions to meet personal needs. Our licensed social worker promotes resident independence, dignity and personal fulfillment.

Monthly Resident Council meetings allow the opportunity for self-expression regarding day-to-day happenings.
